Please choose whichever is appropriate for your John Deere eligible machine (compact utility tractor or residential lawn equipment). This document will need to be downloaded, signed and returned to Cazenovia Equipment when you agree to enroll in the EPIC Warranty program. It can be emailed to service@cazequip.com or faxed to 315-655-8433, ATTN: EPIC CSA Dept.
